<p>Panic disorder is one of several anxiety disorders that involve an intense, abrupt and oftentimes debilitating period of discomfort and fear. Panic disorders affect millions of men and women of all ages, and can be triggered by several situations or circumstances, real or imagined.</p>
<p>Gerald Klerman of the World Psychiatric Association and author of the book, ?Panic Anxiety and Its Treatments? points out that there are several DSM-III-R diagnostic criteria that must be fulfilled in order to classify someone as having <a href="http://panicgoodbye.com/blog">panic attacks</a>.</p>
<p><span id="more-2217"></span></p>
<p>First, one or more of the panic attacks must have occurred when the person was exposed to a situation that doesn?t typically cause anxiety. This could be anything from going to the grocery store, watching a movie they?ve already seen, or washing the dishes. The panic attack must also not be triggered by a situation where the person was the center of attention, such as a public speaking event, a birthday party, or other situation where they were the focus of a crowd.</p>
<p>Secondly, four attacks must have occurred within a four-week period, or the person must have experienced persistent fear after having an attack. Symptoms of fear after a panic attack may include difficulty sleeping, extreme paranoia and restlessness.</p>
<p>Third, during a panic attack the person must also have experienced at least four of the following symptoms: trembling or shaking; sweating; shortness of breath; dizziness; depersonalization; numbness or tingling; hot flashes; fear of dying; chest pain or discomfort; accelerated heart rate; nausea; choking; and fear of going crazy.</p>
<p>Fourth, the person must have experienced at least four of the symptoms in increased intensity within 10 minutes of the beginning of the first symptom they noticed they were having during the panic attack. In many cases, this means the person was feeling very out of control, unbalanced, and even confused.</p>
<p>Finally, the possibility that the attack was triggered by caffeine, amphetamines or hyper-thyroidism must be ruled out. Many people experience the symptoms of panic disorder because they have become dependent on over-the-counter stimulants, or have an unaddressed medical condition. If the person is taking supplements or stimulants, they may not fulfill the criteria for panic disorder by a psychiatrist.</p>
<p>Panic disorder is just one of several anxiety disorders that affect millions of people every year. Panic disorder can become a problem in someone?s life if it interferes with social relationships, or limits the person?s ability to work or live a fulfilling life. <p>A <a href="http://panicgoodbye.com/blog">generalized anxiety disorder</a> can be described as excessive worry about every day things, a feeling that leaves behind a lot of tension and lasts for more than 6 months. This tension and anxiety will interfere with one&#8217;s way of life such that they are not able to function properly. You spend most of the time worrying; you feel impending doom and are overly anxious about things that would not usually worry you. When left unchecked, generalized anxiety disorders can lead to <a href="http://panicgoodbye.com/blog">panic attacks</a>.</p>
<p>Here is how this happens. Panic attacks can generally happen because of a feeling of increased tension and worry in a person. The more you worry, the more your body is convinced that there is some danger, and so starts preparing itself for defence. The typical defence reaction is called fight or flight; the body is getting ready either to fight the enemy, or to run away. It will start to feed the muscles with more energy, the heart will pump a little faster to make sure that more blood is moving amongst the muscles, and the muscles themselves will tighten just in case they have to move into action.</p>

<p>Eventually, a tipping point is reached. All this accumulated energy has to be disposed of. The &#8220;enemy&#8221; is all the fear and the worry that is bottled up inside the body. Without any warning, the body will suddenly react as though it&#8217;s in extreme danger. All the reactions that you would expect in such a situation will manifest; the heart pounds much faster, the muscles will tremble, and the chest will tighten. There is a strong sense of impending doom. The episode lasts for a few minutes and then passes, and it may take a few minutes or even a few days to feel normal again.</p>
<p>Because you are in a state of constant worry, you now have one more thing to worry about: what if I have another panic attack? You will fixate and worry over this to the point where the body starts to prepare all over again to either fight or flee. And then another panic attack occurs, at which point you will now be very worried that something is terribly wrong&#8230;</p>
<p>You can see how generalized anxiety disorders can lead to panic attacks, and panic attacks to generalized anxiety disorders. It becomes a vicious cycle and if you don&#8217;t stop it, it may end up taking over your life.</p>
<p>There are 2 things that you need to look at here. One is the <a href="http://panicgoodbye.com/blog">anxiety disorder</a>. What is causing you to worry endlessly? In many cases, it&#8217;s simply the life that we live. We are caught up in things we do that make us unhappy; we are all too eager to please and so small failures are magnified; we never take a break to refresh and restore ourselves.</p>
<p>The other thing is the panic attacks. Sometimes, they can occur for no reason. It&#8217;s understandable that someone should worry about what could be causing them, but don&#8217;t worry yourself so much that you end up being caught in a spiral of worry that will cause you another attack.</p>

I have panic disorder. My panic attacks happen precisely in closed spaces, like classrooms. I KNOW I will have a panic attack during my exams since I&#8217;ll have to be closed in a classroom 3 hours without permission to leave, in a hot place full of people. </p>
<p>What to do? I&#8217;m thinking of taking meds but then I&#8217;ll be VERY sleepy and not able to focus, plus meds make me sleepy but they don&#8217;t take the panic attacks away!</p>

<p><i>Answer by Sage</i><br />If your panic disorder is medically verified you can get accomodations for your condition during exams. It&#8217;s the law (at least in the US). They would have to provide breaks &#038; etc. It&#8217;s just how it is.</p>
<p>If your panic disorder is not medically verified you have less options available to you. You already know you cannot take the drugs. This is something you already answered in your own post. You will have to work on techniques to cope with your condition if this is the case. I would suggest researching various stress management techniques. No one is saying it&#8217;s not difficult, but without a formal diagnosis/medical verification you are on your own.</p>

<p>There are two treatments for <a href="http://panicgoodbye.com/blog">panic attacks</a> &#8211; medication and psychological therapies. Some people advocate the former while others support the former; some insist on a combination of the two. It all gets a bit confusing so how do you know which treatment course to follow?</p>
<p>First of all, there is no single approach that suits everybody when it comes to panic attacks and anxiety disorders. A cornerstone of effective therapy is that it is individually tailored to the person after a detailed study of the symptoms and when they occur. This should apply regardless of which panic attack treatment you choose to follow &#8211; and, in the end, it is your choice and you have to feel comfortable with it.</p>

<p>One of the most effective psychological approaches is that of cognitive behavioral therapy or CBT. Studies have shown CBT to be more successful in the long run than medication. To be more specific, more people who were treated with CBT remained panic free six months after treatment stopped than those who were on medication. A further study showed that while medication may be effective in treating the symptoms of panic attacks, the benefits stopped when the medication stopped. That is, people had to keep taking the drugs which then lead to unpleasant side effects. </p>
<p>CBT therapy requires work and commitment by the therapist and, more importantly, by the person seeking treatment. It&#8217;s not as easy as taking drugs a few times a day and the effects are not seen so quickly, but if you compare a drug free life free of panic attacks to taking medication for the rest of your life, then the advantages are obvious.</p>
<p>The focus of CBT is getting you to solve the problem yourself so you feel in control of your life again. This is very important as one of the biggest fears of those suffering from panic attacks is that they have lost control over themselves, often to the extent that they feel they are losing their minds. The &#8216;cognitive&#8217; part of the therapy means changing the way you think through a very gradual desensitization process.</p>
<p>For example, let&#8217;s say your first attack occurred in a crowded room, at a party perhaps. Because the feelings were so intense and you were so frightened, you never want to experience it again. So, you go out of your way to avoid that situation &#8211; you make up excuses so as not to face a reoccurence. But this means that you are sitting at home by yourself when everyone else is at the party having fun. Not an ideal situation.</p>
<p>So, desensitization means taking very small steps firstly to get you used to the idea of going to a party (cognition) and then getting you there (behavior). Your first step might be getting dressed as if you are going. That&#8217;s it. Just dealing with the feeling that you are going is enough for one day. This may go on for several days until you feel comfortable with the thought that you are going to a party. When you are ready, you take the next step which may be getting dressed and then opening the front door as if you are on your way out. That&#8217;s it for the second step. When you&#8217;re ready, the third step is getting dressed, opening the front door and closing it. And so on.</p>
<p>While you are going through these steps at your own pace, you are also taught how to deal with the conflicting feelings you have. Controlling your breathing is instrumental here. Rapid breathing is often the catalyst for the other symptoms you experience during a panic attack so if you have techniques for keeping your breathing rate steady then this could be the difference between a slight twinge of anxiety and a full-blown attack.</p>
<p>Many therapists agree that 10-12 sessions are sufficient for most people to be able to manage their problem on their own. After the first five or six, improvement can be seen and the person begins to feel confident that they can regain control of their lives.A final step in the treatment process is to teach management techniques in case panic threatens to rear its ugly head in the future. Armed with this knowledge, it&#8217;s a very real possibility that panic attacks are a thing of the past.
